to notify Alternative Staffing ASAP of all injuries of our employees
that are
injured at your facility! In order to ensure that their medical
treatment will be paid for by your employer or the insurance company,
the employee must select from the physicians or other health care providers
listed on the links below.

employee sign on of the Acknowledgement forms listed below and mail or
